Transitions are rarely linear; expect detours and setbacks. Address the emotional side of change; people need support. Design multiple pathways, not just one. Consider system-level barriers. Test interventions with real users. Plan for long-term maintenance; change isn't a one-time event.

  1. Define the Scope: What transition are we mapping? Who is experiencing it? What's the timeframe?

  2. Map the Current State: Document current behaviors, capabilities, resources, pain points, and system factors.

  3. Define the Desired Future State: Outline target behaviors, required capabilities, success indicators, and resources.

  4. Map Transition Phases: Identify phases like awareness, preparation, action, and integration.

  5. Identify Barriers: Determine capability gaps, motivational challenges, environmental obstacles, and system resistance.

  6. Design Support: Create interventions for capability building, motivation, and environmental/system changes.

  7. Plan Implementation: Develop a detailed transition plan.

  8. Monitor and Adapt: Track progress and adjust the plan as needed.
